The Maryland Cancer Registry (MCR) is operated in accordance with Maryland State Law(Health-General Article, §18-203 and 18-204, Annotated Code of Maryland), and Centers for Disease Control and Prevention (CDC), National Program of Cancer Registries (NPCR), and North American Association of Central Cancer Registries (NAACCR) standards and guidelines. This User Access Agreement describes the terms and conditions under which the MCR WebPlus software may be accessed by reporters and MCR affiliates. WebPlus is part of Registry Plus TM, a suite of publicly available free software programs for collecting and processing cancer registry data, developed and supported by the National Program of Cancer Registries. MCR provides WebPlus access to reporters and MCR affiliates to abstract cases, upload files, and download files. By requesting and receiving approval to access the MCR’s WebPlus system:

1. I understand access to this system is restricted to authorized users and that unauthorized use of or access to this resource may subject me to disciplinary action, up to and including termination of employment, and/or criminal prosecution. If you are not authorized to access this resource, please LOG OFF IMMEDIATELY.

2. I agree that MCR Web Plus will only be used or accessed for MCR-related activities.

3. I understand that my WebPlus and/or MCR security credentials (username and password) must not be shared with anyone.

4. I agree to immediately report to MCR any disclosure or potential disclosure or unauthorized use of my login credentials as well as any unusual activity in my account.

5. I agree to immediately report to MCR any loss of service or abnormalities in MCR WebPlus operation.

6. I agree not to attempt to circumvent, hack, or reverse engineer the MCR software. MCR reserves the right to revoke credentials of any user thought to be abusing the system.

7. I understand that there are state and federal laws and regulations that ensure the confidentiality and security of personal information (PI), personal identifiable information (PII), and protected health information (PHI).

8. I understand that it is a breach of information security and data privacy to use or disclose PI, PII, or PHI in a manner not expressly necessary to complete MCR-related job duties. I will immediately report to MCR any such use or disclosure that I become aware of.

9. I attest that I have received HIPAA and Information Security training within the last year and understand my obligations to safeguard the data within this system.

10. I agree that I am a current employee of my facility, and I am authorized to access the data contained within this system.
